Your Uncle, Phillip L Schamburg of Sacramento, CA has gone on a retreat to Seattle, Washington.  He forgot to have a form notarized, so, he asks his brother, Don Williams-your other uncle, to bring the form to you and have it notarized.  Uncle Don saw Uncle Phillip sign the document.

      Use a Jurat and place Uncle Don under oath-affirming that Uncle Phillip
           requested him to have the document notarized.

      Use a Proof of Execution by Subscribing Witness Jurat and place Uncle Don under oath affirming that Uncle Phillip requested him to have the document notarized.

      Use an acknowledgment-A Jurat is not required as you have personal     
           knowledge of both parties.
